Pengelolaan Anestesi Tanpa Opioid pada Operasi Celah Bibir dan Langit-Langit Laporan Kasus
Abstract
The use of opioids has long been used in general anaesthesia and pain management in children. Side effects of opioids have become an issue in safe anaesthesia care especially in paediatric patients. Cleft lip and palate is a congenital anomaly of the craniofacial region that is most commonly found in the world and in Indonesia. Cleft lip and palate surgery is one of the surgical procedures performed in early childhood. Children with cleft lip and palate have airway abnormalities that put them at risk for respiratory depression. This report aims to determine the effectiveness of opioid-free anaesthetic management in cleft lip and palate surgery. Two paediatric patients underwent cleft lip and palate surgery with general anaesthesia technique combined with
infraorbital nerve block in cleft lip surgery and palatal nerve block in cleft palate surgery. General anaesthesia was given sevoflurane, propofol, ketamine. Intra and postoperative data showed stable haemodynamics, mild pain intensity, low delirium score. Opioid-free anaesthesia offers an alternative in the management of anaesthesia in paediatric patients undergoing cleft lip and palate surgery to reduce the risk of opioid use.
