Abstract


Latar Belakang: Kanker serviks merupakan kanker kedua terbanyak pada perempuan dengan mortalitas 11,4%. Kemoradioterapi sebagai terapi utama stadium lanjut diketahui menyebabkan perubahan morfologi dan fungsi otot dasar panggul.

Tujuan: Mengevaluasi kejadian disfungsi dasar panggul pasca kemoradiasi pada pasien kanker serviks stadium lanjut lokal.

Metode: Penelitian kohort prospektif pada 50 pasien kanker serviks stadium IIB-IVB yang menjalani kemoradiasi di RSUP Dr Sardjito periode Juni 2021-Juni 2022. Evaluasi menggunakan kuesioner PFDI, MOS, dan perineometri.

Hasil dan Pembahasan: Prevalensi disfungsi dasar panggul meningkat dari 32% menjadi 74% pasca kemoradiasi. Terdapat peningkatan signifikan nilai PFDI (9,16 menjadi 24,58), penurunan MOS (3,92 menjadi 2,82), dan penurunan perineometri (11,21 menjadi 8,87) (p<0,05). Relative risk kemoradiasi terhadap disfungsi dasar panggul sebesar 1,62 (p<0,05).

Kesimpulan: Kemoradiasi meningkatkan prevalensi dan memperburuk gejala disfungsi dasar panggul pada pasien kanker serviks stadium lanjut lokal.
