Pengaruh Medication Therapy Management terhadap Kadar HbA1c Pasien Diabetes Melitus yang Mendapatkan Terapi Insulin

Abstract
Pasien diabetes melitus yang mendapatkan terapi insulin rata-rata mempunyai kadar HbA1c relatif tinggi. Kurangnya pengetahuan dan kepatuhan pasien terkait terapi insulin dapat menjadi menyebab terjadinya hipoglikemia yang dapat memperburuk kondisi klinik pasien. Medication Therapy Management merupakan sebuah model intervensi apoteker berkolaborasi dengan tenaga kesehatan lainnya untuk mengatasi permasalahan terkait obat dan meningkatkan outcome pasien secara individu. Tujuan dari penelitian ini adalah untuk mengetahui pengaruh penerapan medication therapy management terhadap kadar HbA1c pasien diabetes melitus yang menggunakan terapi insulin. Penelitian ini menggunakan rancangan kuasi eksperimental dengan one group pretest-posttest design dilakukan pada 25 pasien diabetes melitus yang mendapatkan terapi insulin yang rutin berkunjung ke poliklinik RSUD Toto Kabila Gorontalo dan memenuhi kriteria inklusi. Pasien mendapatkan intervensi pelayanan berbasis medication therapy management dan dilakukan pengukuran kadar HbA1c pre dan post intervensi. Analisis perubahan kadar HbA1c pre dan post intervensi menggunakan uji paired t-test. Setelah pelayanan berbasis MTM terjadi penurunan kadar HbA1c dengan rata-rata 10,93 ± 2,22 menjadi 9,42 ± 2,54 dengan p = 0,006. Hal ini menunjukkan bahwa penerapan medication therapy management dapat menurunkan kadar HbA1c pasien diabetes melitus yang mendapatkan terapi insulin.
