Perbaikan klinis pascaterapi metilprednisolon untuk kasus Bell’s palsy pada preeklamsia tanpa pemberat

Abstract
Background: The incidence of Bell's palsy increases three times in pregnancy, especially in the third trimester. The relationship between Bell's palsy and preeclampsia is still being studied. This case report discusses a case of Bell's palsy in pregnancy with preeclampsia without severe features, which experienced clinical improvement after being treated with methylprednisolone.
Case Presentation: A 37-year-old woman in the third trimester of multigravida pregnancy complained of weakened right facial muscles. She exhibited right cranial nerve VII paresis, lagophthalmos, lacrimation for a day, and a week-long headache rated six on the Numeric Pain Rating Scale (NPRS). No extremity weakness was observed. Her diagnosis was preeclampsia without severe features and Bell's palsy. She received a tapered course of methylprednisolone 62.5 mg three times a day. After four days getting methylprednisolone treatment, the improvements were noted in lagophthalmos, lacrimation, cranial nerve VII paresis, and headache NPRS score.
Conclusion: Bell's palsy, typically caused by swelling or idiopathic factors, is a common reason for facial nerve palsy. Bilateral Bell's palsy during pregnancy is rare and could indicate preeclampsia. Corticosteroid treatment, like methylprednisolone, is the most effective within 72 hours of onset. This case highlights that methylprednisolone therapy improved clinical outcomes in pregnant women with Bell's palsy and preeclampsia without severe features.
ABSTRAK
Latar Belakang: Insidensi Bell’s palsy meningkat tiga kali lipat pada kehamilan, terutama pada trimester ketiga. Hubungan antara Bell’s palsy dan preeklamsia masih dipelajari hingga kini. Laporan kasus ini membahas tentang Bell’s palsy pada ibu hamil dengan preeklamsia tanpa pemberat yang mengalami perbaikan klinis setelah diterapi dengan metilprednisolon.
Laporan Kasus: Wanita 37 tahun G3P2A0 hamil trimester 3 datang dengan keluhan kelemahan otot wajah sebelah kanan. Pada pemeriksaan didapatkan paresis nervus VII kanan, lagoftalmus dan lakrimasi mata kanan onset hari pertama serta nyeri kepala skor enam pada Numeric Pain Rating Scale (NPRS) onset satu minggu. Tidak ada kelemahan ekstermitas. Pasien didiagnosis preeklamsia tanpa pemberat dengan Bell’s palsy dan mendapatkan terapi metilprednisolon 3x62,5 mg tapering-off. Setelah pemberian metilprednisolon hari ke-4 terdapat perbaikan klinis lagoftalmus, lakrimasi, parese nervus VII serta penurunan skor NPRS nyeri kepala.
Kesimpulan: Bell’s palsy menjadi penyebab umum kelemahan otot wajah, dapat disebabkan oleh inflamasi atau idiopatik. Meski jarang terjadi selama kehamilan, Bell’s palsy bilateral diasosiasikan sebagai tanda awal preeklamsia. Terapi kortikosteroid seperti pemberian metilprednisolon paling efektif dilakukan dalam 72 jam pertama onset. Kasus ini menggambarkan terapi metilprednisolon untuk kasus Bell’s palsy pada ibu hamil dengan preeklamsia tanpa pemberat yang menunjukan hasil perbaikan klinis.
