Efektivitas suplementasi Boswellia serrata dan Curcuma longa terhadap intensitas nyeri punggung bawah akut nonneuropatik

Abstract


Low back pain (LBP) is one of disability cause in the world. Non-opioid analgesics and non-steroidal anti-inflammatory drugs (NSAIDs) in long-term use are effective to reduce the intensity of LBP, but cause gastrointestinal bleeding, hypertension, heart failure, and decreased kidney function. Effective and safe alternative therapies for pain management is needed. This study aims to determine the effectiveness of Boswellia serrata and Curcuma longa supplementation to reduce non-neuropathic acute LBP. This study was a double blind randomized controlled trial conducted from July 2020 to March 2021 with 89 subjects analyzed consisting of 44 subjects in the intervention group and 45 subjects in the control group. The intervention group was given a combination of Boswellia serrata 150 mg + Curcuma longa 350 mg/12 hours and paracetamol 500 mg/8 hours for seven days, while the control group took placebo/12 hours and paracetamol 500 mg/8 hours for seven days. The difference in VAS score before and after the intervention was analyzed by independent T-test. Mean reduction in VAS score in the morning, evening, and daily for 24 hours showed that the mean reduction in VAS score at moderate and severe pain intensity was higher in the intervention group than in the control group. In the daily assessment for 24 hours, mean VAS score reduction in severe pain was higher (41.14 mm) than the control group (26 mm) (p =0.024), and for moderate pain, the mean decrease in VAS score in the intervention group was higher (20.32 mm) compared to the control group (17.05 mm, p =0.049). The combination of Boswellia serrata and Curcuma longa supplementation can be considered for moderate and severe intensity of non-neuropathic acute LBP.

 

ABSTRAK

Nyeri punggung bawah merupakan penyebab disabilitas nomor satu di dunia. Analgetik nonopioid dan obat antiinflamasi nonsteroid (OAINS) pada penggunaan jangka panjang efektif menurunkan intensitas nyeri punggung bawah, tetapi menyebabkan perdarahan gastrointestinal, gagal jantung, dan penurunan fungsi ginjal. Terapi alternatif yang efektif dan aman untuk manajemen nyeri sangat dibutuhkan. Penelitian ini bertujuan untuk mengetahui efektivitas penambahan kombinasi Boswellia serrata dan Curcuma longa dalam menurunkan intensitas nyeri punggung bawah akut nonneuropatik. Penelitian dilakukan dengan metode uji acak tersamar ganda dengan pengambilan sampel dari bulan Juli 2020 hingga Maret 2021 dengan 89 subjek yang terdiri dari 44 subjek kelompok intervensi dan 45 subjek kelompok kontrol. Kelompok intervensi mengkonsumsi kombinasi Boswellia serrata 150 mg+Curcuma longa 350 mg/12 jam serta parasetamol 500 mg/8 jam selama tujuh hari, sedangkan kelompok kontrol mengkonsumsi plasebo/12 jam dan parasetamol 500 mg/8 jam selama tujuh hari. Perbedaan nilai VAS sebelum dan setelah intervensi dianalisis dengan independent T-test. Rerata penurunan nilai VAS pagi hari, malam hari, dan harian selama 24 jam menunjukkan penurunan nilai VAS pada intensitas nyeri sedang dan berat lebih tinggi pada kelompok intervensi dibanding kelompok kontrol. Pada penilaian harian selama 24 jam, rerata penurunan nilai VAS pada nyeri intensitas berat lebih tinggi (41,14 mm) dibanding kelompok kontrol (26 mm, p =0,024), dan pada nyeri intensitas sedang, rerata penurunan nilai VAS pada kelompok intervensi lebih tinggi (20,32 mm) dibanding kelompok kontrol (17,05 mm, p =0,049). Penambahan kombinasi Boswellia serrata dan Curcuma longa dapat dipertimbangkan untuk nyeri punggung bawah akut nonneuropatik dengan intensitas sedang dan berat.
