Pengaruh Pengambilan Guta Perca pada Preparasi Saluran Pasak Menggunakan Plugger yang Dipanaskan, Gates Glidden Drill dan Kloroform yang Dikombinasikan dengan Reamer 108 Terhadap Kebocoran Apikal

Abstract


Pengambilan guta perca pada preparasi saluran pasak dapat menyebabkan guta perca mengalami perubahan morfologi atau bahkan perubahan posisi sehingga menyebabkan kemungkinan terjadinya kebocoran apikal. Penelitian ini bertujuan untuk membandingkan teknik pengambilan guta perca menggunakan plugger yang dipanaskan, Gates glidden drill dan klorofom yang dikombinasikan dengan reamer terhadap kebocoran apikal.
Subjek penelitian menggunakan 18 gigi premolar mandibula. Gigi dipotong mahkota anatomisnya, saluran akar dipreparasi dengan teknik crown down dan diobturasi dengan teknik kondensasi lateral. Subjek penelitian dibagi menjadi tiga kelompok masing-masing 6 gigi, kemudian dilakukan pengambilan guta perca untuk preparasi saluran pasak menggunakan a) plugger yang dipanaskan, b) Gates glidden drill dan c) kloroform yang dikombinasikan dengan reamer. Saluran pasak diisi dengan resin komposit, kemudian permukaan luar akar gigi dilapisi cat kuku. Akar gigi direndam dalam larutan biru metilen 2% selama 48 jam, kemudian dipotong longitudinal menghasilkan bagian bukal dan lingual. Pengamatan kebocoran apikal dilakukan dibawah stereomikroskop dengan perbesaran 60 kali.
Hasil penelitian menunjukkan bahwa pengambilan guta perca pada preparasi saluran pasak menggunakan plugger yang dipanaskan menunjukkan tingkat kebocoran apikal yang paling kecil apabila dibandingkan dengan preparasi menggunakan Gates glidden drill maupun kloroform yang dikombinasikan dengan reamer.

ABSTRACT
Removing gutta percha during post space preparation can cause the morfological and position changes, which can effect apikal leakage. The aim of this study was to compare the effect of heated plugger, gates glidden drill and combination of chloroform and reamer on the apical leakage, for the purpose of post space preparation.
Eighteen extracted mandibular premolars were used in this study. The crowns were removed and  the canals were prepared with crown-down technique and obturated using lateral condensation technique. The roots were randomly divided into 3 experimental group of 6 teeth each, and then removed gutta percha was made by either a) heated plugger, b) Gates glidden drill or c) combination of chloroform and reamer. Composite resin was filled into all experimental post space preparation, and then the external surface of all roots was coated with nail polish.  All samples were then submerged in 2 % methylene blue dye for 48 hours, then each root was sectioned longitudinally into bcatuccal and lingual.Apical dye leakage in each group was measured using stereomicroscope with 60X magnification.
The result showed that gutta percha removal on post space preparation using heated plugger demonstrated the minimum apical dye leakage among groups.
